Join us for an in-depth exploration of Healthcare Failure Mode and Effect Analysis (HFMEA), a systematic approach designed by the NCPS specifically to identify and eliminate potential system failures before they reach the patient. Unlike reactive methods like Root Cause Analysis, HFMEA empowers healthcare leaders to move from "fixing what happened" to "preventing what could happen".

Led by Dr. Ghada Sharaf El-Deen, this lecture will guide participants through a streamlined five-step process that replaces complex traditional calculations with an intuitive Hazard Scoring Matrix and Decision Tree. Attendees will learn how to leverage multidisciplinary teams to analyze high-risk clinical processes, formulate actionable recommendations, and design robust outcome measures to ensure lasting safety improvements.

Target Audiences: All quality professionals, healthcare managers, Doctors, Pharmacists, Nurses, and allied healthcare practitioners who seek distinguished knowledge and skills in risk management in healthcare settings and are willing to master its interpretation and application.

The event’s duration is TWO (2) Hours. 

This event has a mixed medium of instruction in both English and Arabic languages.

By the end of this event you will be able to:

  1. Define HFMEA and distinguish it from reactive methods like Root Cause Analysis (RCA).
  2. Identify the five steps of the HFMEA process (Define Topic, Assemble Team, Graphical Description, Hazard Analysis, and Actions/Outcomes). 
  3. Explain the importance of a multidisciplinary team in conducting a successful safety analysis.
  4. Develop a process flow diagram to graphically describe a high-risk clinical process (e.g., medication administration or surgical site preparation).
  5. Conduct a Hazard Analysis by identifying potential failure modes and their causes within a specific process.
  6. Utilize a Hazard Scoring Matrix to calculate Risk Priority Numbers (RPN) based on severity and probability.
  7. Apply the HFMEA Decision Tree to determine whether a failure mode requires further action or can be "controlled" through existing processes.
  8. Formulate actionable recommendations to eliminate, control, or accept identified risks.
  9. Design outcome measures to track the effectiveness of the implemented safety changes.
